There are lots of PHP form to email programs available, some are free. Problem with some is they are prone to have "bots" send spam through them.

Look for one with CAPTCHA capabilities. Will save you headaches later on.

there is many ways of making a php form, i use one witch i can edit through administration but this one is easy one and quick one so it isint that bad after all

I personally don't write php so when I have a need I use Fantastico. However, I have never successfully linked to MySQL. I don't have a clue in this department.
